Make a Behavioral Health Referral

This form allows schools, providers, caregivers, and partners to refer children and youth for community behavioral health services.

What to Expect

Everything you need to know before services begin, including guidance, timelines, and who will connect with you.

Once your referral is submitted, our team reviews the information and determines next steps. Our intake specialist or leadership team will reach out to gather additional details and discuss service options. Have questions about the form? Reach out to the Necco office closest to you.

We always strive to get back to you as soon as we are able. In most cases, families or referral partners can expect follow-up within a few business days after submitting a referral. Response times may vary based on service needs and availability.

Wait times depend on service type, location, and availability. If a waitlist exists, our team will share expected timelines and discuss interim support options when available. Our CBH team strives to initiate services as soon as possible and will work to link you with a provider to avoid waitlists for services.

Many of our services are covered through Medicaid, commercial insurance, private pay, and OhioRISE. Our team will confirm eligibility and coverage during the follow-up process.

Community Behavioral Health (CBH) protects youth and family privacy in accordance with HIPAA and other applicable laws. Information submitted through this referral form is used only to review eligibility and coordinate services and is accessed only by authorized staff and providers. CBH cannot provide the referral source with detailed clinical information, treatment updates, or ongoing status reports without appropriate consent from the youth’s parent/guardian or legal representative. By submitting this referral, you understand that feedback to the referral source may be limited (for example, confirmation that the referral was received or whether the youth was accepted or referred elsewhere) in order to protect the youth’s confidentiality.
